Initial assessment

You are a Registered Psychotherapist at a children's and youth mental health centre in Ottawa. The centre offers a first appointment within a fortnight; after that there is a waitlist of several months for ongoing therapy. Your work is family-systems informed. Ayaan Warsame is fifteen. His mother, Hodan, booked the appointment after the school's guidance counsellor raised concerns about his attendance and about a drawing he handed in. Hodan arrives with Ayaan and with her eldest daughter, who is twenty-four and says she has come to interpret for her mother. Hodan would like to sit in, and adds that Ayaan 'won't tell you anything anyway'. Ayaan has not spoken since he sat down. He is looking at the floor with his hood up, and when you greet him he shrugs. Hodan says, 'Just tell me what is wrong with him and what I need to do.' She mentions that the school has asked for 'a note from the therapist', and that she has taken the morning off work unpaid to be here.
