You are a Registered Psychotherapist in a four-person group private practice in Windsor, and you are the health information custodian for your own clients' records. Hoang Nguyen, 47, has seen you for eleven months - first with his wife for three couple sessions, then individually - following an assault at work. He has been off on a long-term disability claim since February. Three things land in the same week. Hoang's disability insurer writes asking for 'a full narrative report including all clinical notes', enclosing a consent form he signed eight months ago when the claim opened. Hoang emails you asking for a complete copy of his file, saying his lawyer has asked for it. And a colleague in your practice, who covered one of Hoang's sessions in March while you were away, asks you to 'just add a line' to the chart confirming she went through a safety plan with him that day - she is certain she did, but cannot find her note. You have also accepted a position in Ottawa. You intend to close the Windsor practice in ten weeks and have not yet told any of your clients. Hoang's next session is tomorrow morning.
